Extra Details
- This is not dental insurance, but a discount program to reduce out-of-pocket expenses.
- Program is for one year and begins on the enrollment date.
- Payments are made monthly or yearly.
- Program automatically renews unless a written request to cancel is received prior to renewal date.
- Payment for additional dental services is the patient’s responsibility and is due on the date of service to qualify for discounts.
- Program NOT VALID in combination with dental insurance of any kind.
- Program can be cancelled at any time but no refunds will be provided for dues paid, including failure to schedule and maintain appointments.
